Available once a character is bloodied (at half or fewer max HP). On your turn, you can pre-mark a number of death saving throw failures in exchange for an immediate benefit. Marked failures stay on your sheet until you finish a long rest. You cannot have more than 3 death saving throw failures marked at once. If you drop to 0 HP while already at 3 marked failures, this is an instant death; no death saving throws are made.
| Cost | Benefit (pick one) |
|---|---|
| 1 death save failure | Extra Dash and Disengage as Bonus Action |
| 1 death save failure | +5 bonus to a d20 Test you just failed |
| 1 death save failure | Reroll a failed Attack Roll |
| 2 death save failures | When you hit with an attack, each Damage Die deals maximum damage instead of rolling |
| 3 death save failures | Extra Attack or Magic Action |
| 3 death save failures | Regain one expended Spell Slot (level 1–5) |
Death Saving Throw Options
While making death saving throws at 0 HP, each round you have three options:
| Option | Result |
|---|---|
| Roll and succeed | Choose to give an ally within sight Heroic Inspiration instead of counting it as a personal success, or stabilize yourself if you currently have zero death save failures |
| Roll and fail | No Heroic Inspiration |
| Skip the roll | Automatically fail, but give an ally within sight Heroic Inspiration |
